Reflex Sympathetic Dystrophy (RSD), also known as Complex Regional Pain
Syndrome (CRPS), is a chronic pain disorder that currently has no cure.
In addition to severe pain, those with the neurological disorder can
experience abnormal sweating, swelling in tissues, discoloration of skin,
muscle atrophy and sensitivity to light and touch. The most common method
for helping RSD patients manage the pain is through medication. The cycle
of pain management offers some sufferers relief, while others struggle to find
a way to cope with the daily effects of living with pain.<br />
<br />
Hyperbaric oxygen therapy (HBOT) has offered many RSD sufferers an alternative
or supplement to their current medicated pain management regiments. I
have been a practicing neurologist for over 20 years and discovered HBOT when I
was researching therapies to help a family member who had suffered a stroke.
I saw the benefits HBOT can provide firsthand and am passionate about the
healing potential of this type of therapy. We have been providing HBOT to
RSD patients in our Palm Harbor, FL clinic for the past 8 years. Some
patients notice an easing of symptoms within just a few sessions of HBOT.

<b>What Does HBOT Offer the RSD Patient?</b><br />
It can offer a pain reduction cycle that does not require narcotics or needles.
It is safe, cost-effective and painless. During HBOT sessions, patients
breathe 100% oxygen at a higher than normal air pressure. This increased
level of oxygen circulates through the bloodstream and helps decrease swelling

Therapy Could Help Diabetics?<br />

Two months ago, a surgeon told Thelma Bell that wounds on her right foot had
progressed to gangrene and she was facing amputation. She'd already lost two
toes because of nerve and circulatory damage caused by diabetes. </span></div>

<span style="font-family: "arial" , "sans-serif"; font-size: 10.0pt;">But last week, Bell's wounds were
nearly healed, and the tissue on her foot appeared pink and healthy. Doctors
now say her foot and leg will be saved, and once she is fitted with a special
shoe, she should be able to walk normally. </span></div>

<span style="font-family: "arial" , "sans-serif"; font-size: 10.0pt;">Bell escaped the fate of the
estimated 86,000 people each year who have lower limb amputations because of
diabetic complications; they account for half of all leg and foot removals
performed annually in the U.S. Her outcome was different because of wound care
aided by hyperbaric oxygen therapy, says Jeffrey Stone, DO, MPH. </span></div>
<div class="MsoNormal" style="line-height: normal; mso-margin-bottom-alt: auto; mso-margin-top-alt: auto;">
<span style="font-family: "arial" , "sans-serif"; font-size: 10.0pt;">Stone, director of the hyperbaric
medicine unit at the Institute for Exercise and Environmental Medicine (IEEM),
is studying whether use of the therapy can reduce diabetic amputations.
"One of my questions has been the role of hyperbarics in diabetic
wounds," he tells WebMD. He received a $208,000 grant from the American
Diabetes Association (ADA) to investigate the method. </span></div>

<span style="font-family: "arial" , "sans-serif"; font-size: 10.0pt;">Patients are put in a large,
submarine-like chamber and don a clear plastic hood into which 100% oxygen is
pumped. This increases the amount of oxygen in their blood, which in turn helps
generate growth of vessels, says Stone, who also is a physician with the Wound
Care Clinic of North Texas, part of Presbyterian Hospital of Dallas. </span></div>

<span style="font-family: "arial" , "sans-serif"; font-size: 10.0pt;">Vessel growth is crucial for
diabetics because they have circulatory problems due to blocked arteries and
capillaries. They also have nerve problems, called sensatory neuropathy, so
they often can't feel cuts on their feet. "I've had patients come in with a
nail in a foot and not know it," Stone says. </span></div>

<span style="font-family: "arial" , "sans-serif"; font-size: 10.0pt;">Once an injury occurs, it may not
heal if there's not enough oxygen-enriched blood reaching the area. Often, as
in Bell's case, the wound may fester until the only option is amputation, at a
cost of about $40,000 per case. </span></div>

<span style="font-family: "arial" , "sans-serif"; font-size: 10.0pt;">For military veterans alone, this
results in about 9,000 amputations annually at a total cost for surgery,
hospitalization, medical care, and rehabilitation of $341 million, according to
the Department of Veterans Affairs. Diabetics are 15 to 40 times more likely to
have a leg amputated than someone not suffering from the disease. </span></div>

<span style="font-family: "arial" , "sans-serif"; font-size: 10.0pt;">Diabetics and others also must cope
with hardening of the arteries, a condition usually treated by angioplasty,
bypass, or stent placement. But there also are microvascular changes -- clogged
capillaries that occur in the feet of many diabetics. "I think it's these
folks that are helped by hyperbarics," Stone says. </span></div>

<span style="font-family: "arial" , "sans-serif"; font-size: 10.0pt;">"It's important to point out
that hyperbarics is not a panacea. It isn't. There are many other things we do

in March, I thought that they just took you out in the field and shot
you," Bell jokes. "But now my surgeon says that my foot has been
saved." </span></div>
<div class="MsoNormal" style="line-height: normal; mso-margin-bottom-alt: auto; mso-margin-top-alt: auto;">
<span style="font-family: "arial" , "sans-serif"; font-size: 10.0pt;">So far, Stone has treated about 30
diabetes patients under the ADA grant. He wants to treat a total of 100 to 120
people in the double-blind study, in which neither medical personnel nor
patients know whether the participant is receiving pure oxygen or regular air
during their 90 minutes to two hours in the chamber. </span></div>
<div class="MsoNormal" style="line-height: normal; mso-margin-bottom-alt: auto; mso-margin-top-alt: auto;">
<span style="font-family: "arial" , "sans-serif"; font-size: 10.0pt;">Hyperbaric medicine is not new. It
has been used since the 1940s to treat decompression sickness from scuba
diving, carbon monoxide poisoning, and chronic bone infections. For the past 35
years, it also has been used for healing wounds. Stone also previously used it
for patients with diabetes, but this is the first time a double-blind study has
been done to determine if the treatment is effective enough to significantly
reduce amputations resulting from diabetic ischemic foot ulcers, or wounds not
getting sufficient oxygenated blood for healing. </span></div>
<div class="MsoNormal" style="line-height: normal; mso-margin-bottom-alt: auto; mso-margin-top-alt: auto;">
<span style="font-family: "arial" , "sans-serif"; font-size: 10.0pt;">In an earlier retrospective study,
Stone and his colleagues looked at results for 1,633 patients treated for
wounds over 33 months. Of those, 501 were diabetic; 119 received hyperbaric oxygen
therapy and the rest received conventional treatment. They found that the limb
salvage rate was 72% for those on pure oxygen and only 53% for the rest. Stone
cautions that this was not a controlled study, so not a true measure of the
effectiveness of hyperbarics in reducing amputation. </span></div>
<div class="MsoNormal" style="line-height: normal; mso-margin-bottom-alt: auto; mso-margin-top-alt: auto;">
<span style="font-family: "arial" , "sans-serif"; font-size: 10.0pt;">Ben Gallegos, administrator of the
hyperbarics unit at Medical City Dallas Hospital, says that though they haven't
conducted an official study of the use of the therapy in diabetics, they have
had great success in using it against gangrene. Medical City is the only other
facility besides Presbyterian in the Dallas area that has a multichamber
hyperbaric unit. </span></div>
<div class="MsoNormal" style="line-height: normal; mso-margin-bottom-alt: auto; mso-margin-top-alt: auto;">
<span style="font-family: "arial" , "sans-serif"; font-size: 10.0pt;">Roger Unger, MD, who is not involved
in the ADA study, also expressed caution. He is director of the Touchstone
Diabetes Center at UT Southwestern Medical Center at Dallas, and a diabetes
clinician at the Dallas Veterans Affairs Medical Center. </span></div>
<div class="MsoNormal" style="line-height: normal; mso-margin-bottom-alt: auto; mso-margin-top-alt: auto;">
<span style="font-family: "arial" , "sans-serif"; font-size: 10.0pt;">"We have sent patients for
hyperbaric treatment," says Unger, a member of the National Academy of
Sciences (NAS). "Diabetic foot ulcers and amputation is a terrible
problem, and almost anything you do fails. </span></div>
<div class="MsoNormal" style="line-height: normal; mso-margin-bottom-alt: auto; mso-margin-top-alt: auto;">
<span style="font-family: "arial" , "sans-serif"; font-size: 10.0pt;">"At this point, no one knows
how effective hyperbaric treatment is, so it's important to do such a study.
We've seen temporary improvement from hyperbarics, but it's hard to tell
whether it's because of that therapy or because other treatment the patient is
receiving is focused on the wound," Unger says. </span></div>
<div class="MsoNormal" style="line-height: normal; mso-margin-bottom-alt: auto; mso-margin-top-alt: auto;">
<span style="font-family: "arial" , "sans-serif"; font-size: 10.0pt;">Jean Wilson, MD, former chief of
endocrinology at UT Southwestern who has treated numerous cases of diabetic
wounds and amputation at Parkland Health and Hospital System, also expresses
skepticism but commends IEEM for doing the study. </span></div>
<div class="MsoNormal" style="line-height: normal; mso-margin-bottom-alt: auto; mso-margin-top-alt: auto;">
<span style="font-family: "arial" , "sans-serif"; font-size: 10.0pt;">"We need effective therapies,
and I'm enthusiastic that the ADA is funding such research," says Wilson,
also an NAS member. "It's very important that hyperbaric medicine be
studied because its effectiveness for most of its applications has never been
scientifically investigated." </span></div>
<div class="MsoNormal" style="line-height: normal; mso-margin-bottom-alt: auto; mso-margin-top-alt: auto;">
<span style="font-family: "arial" , "sans-serif"; font-size: 10.0pt;">Stone says that in the retrospective
study, they found that patients accepted for hyperbarics had larger wounds,
more wounds, and were more likely to have been recommended for amputation than
the rest of the people receiving wound care. </span></div>

<span style="font-family: "arial" , "sans-serif"; font-size: 10.0pt;">"Interestingly, 31% of the
hyperbaric group had been recommended for amputation versus 19% of those who
did not have hyperbarics," he says. "Based on this, we applied for
the ADA grant. </span></div>

currently affecting as many as 1 out of 166 children in the United States.
Numerous studies of autistic individuals have revealed evidence of cerebral
hypoperfusion, neuroinflammation and gastrointestinal inflammation, immune
dysregulation, oxidative stress, relative mitochondrial dysfunction,
neurotransmitter abnormalities, impaired detoxification of toxins, dysbiosis,
and impaired production of porphyrins. Many of these findings have been
correlated with core autistic symptoms. For example, cerebral hypoperfusion in
autistic children has been correlated with repetitive, self-stimulatory and
stereotypical behaviors, and impairments in communication, sensory perception,
and social interaction. Hyperbaric oxygen therapy (HBOT) might be able to improve
each of these problems in autistic individuals. Specifically, HBOT has been
used with clinical success in several cerebral hypoperfusion conditions and can
compensate for decreased blood flow by increasing the oxygen content of plasma
and body tissues. HBOT has been reported to possess strong anti-inflammatory
properties and has been shown to improve immune function. There is evidence
that oxidative stress can be reduced with HBOT through the upregulation of
antioxidant enzymes. HBOT can also increase the function and production of
mitochondria and improve neurotransmitter abnormalities. In addition, HBOT
upregulates enzymes that can help with detoxification problems specifically
found in autistic children. Dysbiosis is common in autistic children and HBOT can
improve this. Impaired production of porphyrins in autistic children might
affect the production of heme, and HBOT might help overcome the effects of this
problem. <b>Finally, HBOT has been shown to mobilize stem cells from the bone
marrow to the systemic circulation.</b> Recent studies in humans have shown
that stem cells can enter the brain and form new neurons, astrocytes, and
microglia. It is expected that amelioration of these underlying
pathophysiological problems through the use of HBOT will lead to improvements
in autistic symptoms. Several studies on the use of HBOT in autistic children
are currently underway and early results are promising.<o:p></o:p></span></div>

tubing is used to deliver carbon dioxide during laparoscopic procedures.
Failure to prime the tubing with carbon dioxide prior to abdominal insufflation
may result in the delivery of nitrogen-containing air to the abdominal cavity.
We report a case in which initial insufflation of laparoscopic gas resulted in
immediate cardiovascular collapse requiring prolonged resuscitation. Persistent
intracranial emboli following the arrest may have resulted from nitrogen
contamination of the delivered gas. CLINICAL FEATURES: A 12-day-old female
underwent laparoscopy for pyloric stenosis. During initial insufflation of the
abdomen, the patient had an abrupt decrease in end-tidal carbon dioxide
(CO(2ET)) associated with bradycardia and pulseless electrical activity. Three
hours after successful resuscitation and open pyloromyotomy, computerized
tomography documented intra-<b>arterial gas within the cerebral and hepatic
circulations that resolved following hyperbaric oxygen therapy.</b> Magnetic
resonance imaging five days later revealed watershed infarcts in the right
frontal and parietal regions. Nitrogen, an insoluble gas not easily eliminated
from the body, was likely the gas present within the patient's circulation
several hours after the event. It was unlikely carbon dioxide, which is a
highly soluble gas that binds to hemoglobin and is rapidly buffered by the
carbonic anhydrase system and excreted by the lung. Room air contamination of high
volume insufflation tubing allows nitrogen to enter body cavities during
endoscopic procedures. CONCLUSION: Persistence of emboli following endoscopic
procedures suggests that the entrained gas is insoluble. Room air contamination
increases the potential for catastrophic events during laparoscopy and other
endoscopic procedures.<o:p></o:p></span></div>

initially developed to combat the adverse effects of deep sea diving. In recent
years, hyperbaric oxygen treatment has been used to treat a broad spectrum of
ailments, including delayed-onset muscle soreness, fibromyalgia, and complex
regional pain syndrome (CRPS). However, limited data are available on the
effect of hyperbaric oxygen treatment on inflammatory pain. The effect of
hyperbaric oxygen treatment on carrageenan-induced inflammation and pain in
rats was investigated. It was hypothesized that treatment with hyperbaric
oxygen would decrease paw edema and hyperalgesia in an acute inflammatory pain
model as compared with that of a sham-treated control group.</span><o:p></o:p></div>

on 44 male Sprague-Dawley rats each between 300 and 350 g. The inflammatory
pain condition was induced by subcutaneous injection of 1% carrageenan
suspended in saline in the left hind paw. Paw volume was assessed via water
displacement with a plethysmometer, and percentage differences were calculated
on the basis of posttreatment measures compared with pretreatment measures.
Hyperalgesia was assessed using the up/down method of mechanical paw withdrawal
thresholds. Hyperbaric oxygen treatment involved exposing animals to 100%
oxygen at a pressure of 2.4 atmospheres absolute (ATA) for 90 minutes in a hyperbaric
chamber. A control group was placed in the hyperbaric chamber but did not
receive treatment.<o:p></o:p></span></div>

analysis of variance revealed no significant preinjec-tion group differences.
An overall mixed-design analysis with 2 group levels (treatment, sham) and 7
levels of time revealed a significant main effect for group, a main effect for
time, and a group by time interaction. In the hyperbaric oxygen treatment
group, paw edema remained at pretreatment levels immediately after treatment
and continued to decrease slightly until 2 hours posttreatment, at which time
it began to decrease. However, antinociceptive effects were apparent
immediately after treatment and continued to increase up to 5 hours after
treatment, suggesting that distinct mechanisms might be involved in the

OBJECTIVE: To detect the feasibility and theoretic basis for treatment with
hyperbaric oxygenation (HBOT) in chronic hepatitis and to compare the changes
in hepatic function, immunity, pathologic morphology, ultra structure and HBV
in hepatic tissues before and after treatment. METHODS: Sixty cases of chronic
hepatitis were randomly selected and divided into two groups: the experiment (n
= 30) and control groups (n = 30). Patients in the experimental group were
treated with HBOT for 6 courses. Patients in the control group were treated for
60 days with the usual drugs used in the clinic. The function and bloodstream
graph of liver were examined and liver biopsies were made before and after
treatments. Routine paraffin sections were stained with HE and observed under
the light microscope. Ultra thin slides from paraformaldehyde and
glutaraldehyde fixed liver tissue were stained with lead citrate and observed
with the transmission electric microscope. HBsAg and HBcAg in liver of the
experimental group were detected with ABC immunohistochemistry method before
and after treatment.<o:p></o:p></span></span></span></div>

<span style="color: windowtext;"><span style="font-size: x-small;"><span style="font-family: Verdana;">RESULTS: For the
experimental group, ALT, SB, gamma-GT, AKP, IgG, and IgM in blood and the
degeneration and necrosis of hepatocytes were remarkably decreased (P < 0.05
), the mean contractive wave of bloodstream in liver and the bloodstream in
right ramus of janitrix were remarkably increased (P < 0.05), and the
swelling of mitochondria, increase of lysosomes, generation of Kupffer cells,
infiltration of lymphocytes in portal area and capillary generation were all
remarkably all eviated (P < 0.05) after treatment with HBOT. There were
significant differences between the experimental and control groups after
treatment with different methods (P < 0.05). For patients in the
experimental group, the fibrosis and fat-storing cells in the liver were not
reduced (P > 0.05), and the expression of HBsAg and HBcAg in liver was not
weakened (P < 0.05) after treatment. <o:p></o:p></span></span></span></div>

effective and recommendable, but it could not reverse liver fibrosis. However,
it might be able to delay or prevent the liver from fibrosis, so it might be
more effective at the early and middle stages of chronic hepatitis. HBOT could
not inhibit the HB virus. So we consider that treatment with HBOT should be
simultaneous with anti HBV therapy.</span></b><span style="color: windowtext;">

sessions has come to be a benchmark in this therapy, not solely in the
treatment of ASD, but rather in treating all conditions characterized by
hypoxic tissue. It is theorized that this is the number of treatments
necessary to ensure angiogenesis has occurred. In the past, physicians
would notice “backsliding” or regression with most patients who had undergone
less that thirty sessions. On the other hand, patients having undergone
forty sessions would not display symptoms of “backsliding”. Today, with
new knowledge regarding increased stem cell activity and sophisticated brain
imaging equipment, it is easy to paint a picture as to what was
occurring. Recent research has demonstrated that after only ten sessions an
eight fold increase in stem cell activity can be measured; in particular, CD34
stem cells which will produce new capillaries. The formation of such
capillaries can be seen between 15-20 sessions and the completion of such
capillaries appears to occur between 30-40 sessions; hence the Magic Number
40. Of course, this process is continually underway and new capillaries
are just beginning at 20 sessions, 30 sessions, 40 sessions, etc. One
would assume that as long as hypoxic tissue remains in the body; stem cell activity
would remain heightened, angiogenesis would continue, and tissue perfusion
would continue to normalize. In fact, in a study conducted with CP,
stroke, and traumatic brain injury patients; it was demonstrated that
throughout 70 sessions the rate of improvement in cerebral oxygenation
increased during the last 35 treatments. For this reason, it is not
uncommon to see treatment protocols continue beyond the initial forty-session
sequence; parents are often advised 60-80 initial sessions prior to pausing,
ending, or lessening the frequency of treatment. <br />

documented the effectiveness of hyperbaric oxygen in models of acute and
delayed crush injury. Intermittent exposure to hyperbaric hyperoxia serves to
interrupt the injury cycle of edema, ischemia and tissue necrosis<sup> (1)</sup>,
as well as hemorrhagic hypotension<sup> (2)</sup>, which in turn leads to
former edema and ischemia. Tissue ischemia is countered by the ability of
hyperbaric oxygen to elevate tissue oxygen tensions<sup> (3)</sup>.
Furthermore, edema is reduced, secondary to hyperoxia-induced arteriolar
vasoconstriction<sup> (4)</sup>, leading to improved tissue viability, thereby
reducing necrosis<sup> (1)</sup>. Hyperbaric oxygen has also been studied in
models of peripheral nerve injury<sup> (5)</sup>. Researchers from the US Air
Force School Aerospace Medicine and Louisiana State University recently sought
to determine what, if any, morphologic changes are associated with hyperbaric
oxygen treated peripheral nerve injury<sup> (6)</sup>. Their model involved a

analysis of electron micrographs. All of the animals exposed to hyperbaric
oxygen were reported to demonstrate advanced stages of a healed nerve, in
contrast to both control groups. As this research was limited to a determination
of regeneration of morphology, the exact effects of hyperbaric oxygen were not
known. The authors speculate, however, that there may be several suggesting
increased myelination, decreased edema, reduced internal collagen and
improvements in neurofilamentous material density. They conclude that this
study provides additional evidence of a link between tissue oxygen levels from

sessions has come to be a benchmark in this therapy, not solely in the
treatment of ASD, but rather in treating all conditions characterized by
hypoxic tissue. It is theorized that this is the number of treatments
necessary to ensure angiogenesis has occurred. In the past, physicians
would notice “backsliding” or regression with most patients who had undergone
less that thirty sessions. On the other hand, patients having undergone
forty sessions would not display symptoms of “backsliding”. Today, with
new knowledge regarding increased stem cell activity and sophisticated brain
imaging equipment, it is easy to paint a picture as to what was
occurring. Recent research has demonstrated that after only ten sessions an
eight fold increase in stem cell activity can be measured; in particular, CD34
stem cells which will produce new capillaries. The formation of such
capillaries can be seen between 15-20 sessions and the completion of such
capillaries appears to occur between 30-40 sessions; hence the Magic Number
40. Of course, this process is continually underway and new capillaries
are just beginning at 20 sessions, 30 sessions, 40 sessions, etc. One
would assume that as long as hypoxic tissue remains in the body; stem cell activity
would remain heightened, angiogenesis would continue, and tissue perfusion
would continue to normalize. In fact, in a study conducted with CP,
stroke, and traumatic brain injury patients; it was demonstrated that
throughout 70 sessions the rate of improvement in cerebral oxygenation
increased during the last 35 treatments. For this reason, it is not
uncommon to see treatment protocols continue beyond the initial forty-session
sequence; parents are often advised 60-80 initial sessions prior to pausing,
ending, or lessening the frequency of treatment. <br />

was initially developed to combat the adverse effects of deep sea diving. In
recent years, hyperbaric oxygen treatment has been used to treat a broad
spectrum of ailments, including delayed-onset muscle soreness, fibromyalgia,
and complex regional pain syndrome (CRPS). However, limited data are available
on the effect of hyperbaric oxygen treatment on inflammatory pain. The effect
of hyperbaric oxygen treatment on carrageenan-induced inflammation and pain in
rats was investigated. It was hypothesized that treatment with hyperbaric
oxygen would decrease paw edema and hyperalgesia in an acute inflammatory pain
model as compared with that of a sham-treated control group.</span></div>

conducted on 44 male Sprague-Dawley rats each between 300 and 350 g. The
inflammatory pain condition was induced by subcutaneous injection of 1%
carrageenan suspended in saline in the left hind paw. Paw volume was assessed
via water displacement with a plethysmometer, and percentage differences were
calculated on the basis of posttreatment measures compared with pretreatment
measures. Hyperalgesia was assessed using the up/down method of mechanical paw
withdrawal thresholds. Hyperbaric oxygen treatment involved exposing animals to
100% oxygen at a pressure of 2.4 atmospheres absolute (ATA) for 90 minutes in a
hyperbaric chamber. A control group was placed in the hyperbaric chamber but
did not receive treatment.</span></div>

variance revealed no significant preinjec-tion group differences. An overall
mixed-design analysis with 2 group levels (treatment, sham) and 7 levels of
time revealed a significant main effect for group, a main effect for time, and
a group by time interaction. In the hyperbaric oxygen treatment group, paw
edema remained at pretreatment levels immediately after treatment and continued
to decrease slightly until 2 hours posttreatment, at which time it began to
decrease. However, antinociceptive effects were apparent immediately after
treatment and continued to increase up to 5 hours after treatment, suggesting
that distinct mechanisms might be involved in the anti-inflammatory and
antinociceptive properties of Hyperbaric Oxygen Treatment.</span></div>
